Galaxy AWell, I'm not sure where you are seeing the "Multimedia" folder, but the app is only 5.69 Meg.
Your images are the Data. As you add more images / videos then you will have more Data.

Galaxy ASo it's the same size as the total size of them, not an additionnal 21 gb ?

Galaxy ANormally clear cache/data force the app to rebuild a new and fresh library, but face in your case maybe I would make a backup before try and see.

Galaxy AYou should be able to clear Cache for the Gallery you are using.
Clearing Data would remove all of your pictures / videos etc, so that is NOT a good idea.
