Galaxy NoteGo to Settings > Notifications after you hear the "over the horizon ringtone". I'm thinking that it's likely to an Android system Notification, that has the ringtone.
You should be able to Turn it Off, by going to Settings > Notifications & look for Android System. Click on it & then you can change the Type of Notifications from "Allow Sound & Vibration" to "Deliver Quietly", then you won't hear the Notification.
You could also use Good Lock's NotiStar to check if something else is causing the Notification.
That's my guess.

Galaxy NoteWell once you see Android System as a recent Notification, you could either turn Android System Notifications off completely (which I would NOT recommend), but you can also look down to see Notification Categories. Here you have to option to Toggle Off specific Android System Notifications.

Galaxy NoteI did not do anything special. So not sure how it got fixed. But I can recommend try clear Phone app data and cache.
Settings → Apps → Storage
Clear data & clear cache
If you have preference for your phone app, you need to set again, like ringtone , spam call settings or show your caller lD...etc. All preference will set to default. Try if this help.

Galaxy NoteUnless I completely misunderstood, I don't think that they are talking about the ringtone for incoming calls.
I think that it's other Notifications, that don't appear to be for something they necessarily want to get, ie: some Android System Notifications?
