Galaxy SLong Press anywhere on an empty part of the home screen & ensure that the desktop is not locked > Open the app drawer & find the phone Icon & long Press on it & Select "add to the home screen".
You'll probably have to long press on it again and drag it to where you want it.
Long Press on an empty part of the desktop & lock the home screen again.

Galaxy SI use Nova Launcher,
But look at the screen shots.
Long Press on an empty space on the desktop > Click on the "Gear" icon for the desktop settings > Toggle OFF lock desktop > Find the phone Icon (I used the clock app & you can see where it says "add to desktop).
Place the icon where you want it & lock the desktop.
