I don't know what that MMGuardian is but I would disable all those and reboot. If it doesn't pop up then turn some on and see if one of those is causing it. First, turn on MMGuardian and reboot just to rule it out

You wrote, "whenever I turn my phone on" ... do you mean after you power up, or after you wake up the phone from sleeping/screen off?

Please try clearing the system cache and rebooting your device.
