Could be by design. 🤷♂️
While I try hard not to drop phones, I have seen this with previous Note9, Note10 and S22U devices.

If you haven't bought a protective case and screen protector already buy them for your model.. Given your statement about every time you drop concerns me. If you haven't got a case already please buy a case on-line. Be careful to buy one specific to the S23 Ultra. I have dropped both my tablet and S23 FE several times where the screen would have broken if it had no protection. Now for an expensive laugh. The only time I broke a screen was when I took the case off to send to Samsung for an internal repair problem. That's usually a non-warranty $400 repair plus no phone for several weeks.

I mean, I am already using a protector and a case just where I drop it, not like a hard drop, just a soft drop. The s pen keeps popping out, and I could lose it.

I suggest a few Google searches including Samsung but with and without your model name. Others have similar problems.

I can't help feeling that there are two issues. The first being ensuring that the s pen comes out when you want it to, and the second stopping it when you don't. What if you stuck a slightly stiff piece of leather on the casing covering the hole but short enough not to block the pinhole at the top. Make it so when you bend the material you can extract the pen normally but stiff enough to stop the s pen falling out. Alternatively a tiny piece of velcro from front to back over the top of the s pen hole from front to back ensuring the velcro doesn't interfere with any camera lens. The other thing to check is if any 3rd party case deals with the problem and replace your existing one.
