I'm assuming this is Samsung Health right? I think this phone might have been rooted or modified. You can check Settings - About Phone - Status - Phone Status - if it says Official, you should be okay. If not, then there is probably some form of modification. Please let us know! if not, you're device was rooted in the pass
You can try to put your phone in recovery mode with this step
Power off your Samsung phone.
Press and hold "Volume UP + Power + Home" button at the same time, and as soon as the phone reboot, release all three buttons and wait to enter the recovery mode.
You can then choose different options offered in recovery mode by clicking Volume Up/Down button or verify your choice by clicking Power button.
and if you see 0x0 your device is official
if you see 0x1 your device was rooted in the past
