Galaxy SYou can try a couple of things. Open the Camera & Settings > Click on the "Gear" & scroll down to Reset the Camera Settings.
Boot into Recovery & Wipe Cache.
See if either of these help.

Galaxy SHi, we are sorry to hear that your'e facing this issue.
Please reset the camera to its default settings by the steps given by RedDogRabbit and verify if it resolves the issue.
You can also try using the camera in Safe Mode to find out if a recently installed app is causing the camera focus problem.
To enter Safe Mode, please perform the following:
1.First, completely power off the phone.
2.Then, power on the phone and when the Samsung logo appears, press and hold down the Volume down key.
3.If done correctly, "Safe Mode" will display on the bottom left corner of the screen. If "Safe Mode" does not appear, repeat the steps above.
4.Check your camera's focus function and if necessary, uninstall any recently installed apps.
5.Restart your phone to exit Safe mode.
