If anyone can help, Samsung can't. Note9, using Samsung email app, since last update in March, I have to restart my email app or clear cache to retrieve my messages. As per Samsung I have removed and reinstalled my account maybe 10 x now. Still doesn't work properly. Factory reset of phone, then restored backup from prior to update, still same problem. Email account is apple icloud. No settings have changed in 10 years+. Any help?

Since you have tried:
- Remove and re-add account
- Clear app cache and data
- Using password generated from iCloud
- Double checked all settings
- Factory reset

None of above suggestions work. I can only suggest the following and try it out when you have time.

Try using MS Outlook app or Gmail app host your iCloud account. See if other Mail app having the same sync problem or not. If other apps have no such problem, that proof that Samaung Email app has IMAP sync problem with iCloud.

Put my account with gmail, gets messages okay, but gmail app doesn't do push notifications for imap, every 15 minutes is the shortest. Have to try outlook.

So gmail works, outlook works, samsung email doesn't.

Obvious Samsung Email apo has glitch after recent update on IMAP sync. In this case, use other app instead is the only solution. But happy to hear that other app works for you.

You might also ve thinking of Auto (when received) under the syncing schedule, which is also on
