I have a Galaxy S21+ and since the last update my settings in DO NOT DISTURB change in the middle of the night every night. I set it manually with Alarms and Media allowed. Calls/texts from fav contacts and for notification panel to show. But I've not been able to rely on that as it keeps resetting itself at some point in the night and my alarms don't work. I've checked it before I go to sleep and when I wake up it's different settings. Please help!

Start by Booting into Recovery & Wipe Cache.

Check the DND settings again & see if it's working properly now?

Instructions on how to Wipe Cache:

1) Turn off the device.

2) Press and hold the Volume Up key and the spower button.

3) When the Android logo displays, release all keys.

4) Press the Volume down key to navigate and select "Wipe cache partition".

5) Press Power key to select. When the wipe cache partition is complete, Select Reboot System.

6) Press the Power key to restart the device.

Hi, please restart your device and check if the Do not disturb is working properly, many minor issues can be fixed by restarting the device.

If you still face the issue with Do not disturb you can perform Wipe cache partition by following the steps provided by RedDogRabbit and verify if it helps to fix the problem.

I have restarted device on multiple occasions. Which seems to help only to reset itself at some point during the night. So perhaps the wipe cache is my next move. Thank you

You could possibly have a routine enabled. I was having the opposite issue of it turning on by random. Turns out Driving mode was enabled so when it connected to my truck Bluetooth, it enabled do not disturb and then wouldn't turn back off after getting out of the truck.
Could be doing the opposite and disabling it as soon as you connect to something
