Galaxy STry the following and see if it is caused by software.
Open Settings → Apps → Phone → Storage
Click clear data and click clear cache
* This will reset your Phone app settings back to default settings.
If you have no problem reset your phone app settings, try it and test your speaker issue again.

Galaxy S- Power off your phone
- Press and hold Vol up and power key until you are in Recovery mode
- Release both keys
- Use Vol up and down to navigate
- Use Power key to confirm
- Select Wipe Cache Partition
- Select Reboot system
Test speaker phone with someone again. Also try testing with different person.
