When did this issue start? Was it right out of the box, or did it have issues after a while?

I know you tried resetting it, but try following this guide - https://www.samsung.com/africa_en/support/mobile-devices/how-do-i-connect-the-s-pen-via-bluetooth/ 

Ensure that the pen also has battery life(thanks @Vic20 and @SnowCat19).
